To all of our Patients & Families,

We hope that you’re all doing well and staying safe. We know that these past few months have been difficult for everyone. We’re happy to announce that we will be re-opening as of June 1^st. We ask that you be patient with us as we have a numerous amount of patients that we need to see and our team of doctors will be reviewing each case individually and giving priority to those the doctor feels need to be seen immediately. However, we know that there are many patients that haven’t been seen in several months and we fully intend to see each of them as soon as we’re able to. The American Association of Orthodontics & the CDC have placed many new protocols that our office must follow. When your appointment time comes you will notice some changes to our appearance and our office protocols. These changes are for the safety of our patients as well as our staff.

1. Prior to your appointment, current patients & new patients please fill out and sign our supplemental health questionnaire and consent forms. These forms can be found on our website and or filled out at the office prior to entering the office. http://www.fivestarorthodontics.com/covid-19-forms.
2. Prior to your appointment we ask that you please floss, brush & use a mouth rinse.
3. You MUST wear a MASK to your appointment and the mask must remain on until the Doctor or Dental Assistant asks you to remove it.
4. It is recommended by the American Association of Orthodontics that only the patients come into the office for their appointment. All parents/guardians & siblings should remain in the car during the appointment time. When you arrive to the office please call the office and let them know you are here for your appointment. Please try to arrive to the office 15 minutes prior to your appointment. After you notify us that you’re here we will then collect the signed consent forms as previously mentioned and a staff member will be taking the patients temperature and asking the patient to use our provided hand sanitizer before entering the office.
5. After the appointment one of our Dental Assistants will accompany the patient out to the car to review the patient’s treatment status.
6. We recommend you call the office to schedule the patients next appointment right away before you leave the parking lot. This is advised to ensure that we can get them in for their next available appointment.
